Based on record review, the licensee did not comply with the section cited above in 4 out of 6 children have no immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Facility staff will immediately report all required incidents to the Director/Owner and to CCL. Staff will be reminded of reporting rules, and the Director/Owner will ensure timely submission of all required reports. POC letter shall be submitted on or before 7/23/2026. during the complaint investigation, it revealed that addidtional concerns regarding former staff # 1 interaction was not reported which poses a potential risk to personal rigts of chldren in care
