Based on interviews, the licensee did not comply with the section cited above when staff failed to adequately supervise C1 when C1 behave inappropriately around other children which poses an potential risk to the health, safety or personal rights of children in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by ensuring sink 2 in room 2 and a sink identified as the kitchen food prep sink did not exceed the acceptable amount of lead allowed in a child care facility, which poses/posed a potential health, safety or personal rights risk to persons in care.

report review and interviews: A child wandered away from the facility without staff intervention. Outside the facility he was not under the care or supervision of day care staff. This poses an immediate risk to the Personal Rights of children in care. 8 Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

staff interview and LPA observation: there are areas at the facility that are not safe where children can exit facility fences and exit into the yard.
