Based on observation interview record review, the licensee did not comply with the section cited above in the facility did not have a child/parent roster created during the inspeciton which poses a potential health, safety or personal rights risk to persons in care.
101223(a)(1): The licensee shall ensure that each child is accorded the following personal rights:(1) To be accorded dignity in his/her personal relationships with staff and other persons. This requirement was not met as evidence by: 1 Teacher #1 will receive training on Redirecting Behavior: How to Deal with Challenging Behavior, and will send proof of completion to LPA Tamayo no later than 09/20/2025 Assistant Teacher #1 did not observe Teacher #1 slap the hand of child #1, however Teacher #1 did pull a book out of child #1 hand in an aggressive manner, which poses a potential health and safety risk to children in care. 8
