The carpets and rugs in the facility are soiled and stained with dirt and need thorough shampooing or replacement/removal. It poses a potential risk to the health, safety, and personal rights of the children in care.

LPA observed that S1 was supervising 13 children in the playground, and S2 was left alone on multiple occasions with 17 children in care, which poses a potential risk to the health, safety, or personal rights of children in care.
