Based on observation, the licensee did not comply with the section cited above in that LPA observed various stains on the walls, various boxes and items stacked upon each other on top of the preschool and toddler classroom's cubbies, and toddler rug held various dark stains, which poses/posed a potential health, safety or personal rights risk to persons in care.
(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. Director, Anna Gonzales stated that girls toliet located in the back of the restroom, also the largest stall, will be fixed. Director stated that a work order form will be provided to the Department showing that the toliet had been repaired. Anna stated that POC will be submitted by 01/12/2024 to the Department. Based on observation, the facility did not comply with the section cited above. In that LPA observed that one toliet out of three leaked water when flushed in the girls restroom, which poses a potential health, safety or personal rights risk to persons in care. 8

The LPA observed the licensee to have cleaned out the back up freezer. The licensee agrees to continue to monitor and check the food storage, especially in the back up freezer, on a regular bases to ensure things are being the LPA observations, further discussed on the 9099 report. This is a possible risk to the health, safety or personal rights of children in care. 8 properly stored and maintaining temperature. The licensee also agrees to train staff on proper storage, temperatures and requirements in the kitchen.

Based on record review, the Administrator did not comply with the section cited above. LPA observed that Staff #1 did not observe a current complete the Mandated Reporter training, which poses a potential health, safety or personal rights risk to persons in care.
Based on observation and interview, the licensee did not comply with the section cited above. Upon arrival LPAs met with Staff #1 who stated they was currently the person in charge. Both Director and Assistant Director were not present. LPAs were unable to review Designation of Facility Responsibility (LIC 308) and other documentation to verify that staff #1 is designatined to act on the Director's absence. This poses a potential health, safety or personal rights risk to persons in care.
Based on observation and interview, the licensee did not comply with the section cited above. Staff #1 stated they were are unable to provide LPAs children and staff records due to records being locked in cabinet and only available to Director via key. LPAs were unable to review records which poses a potential health, safety, or personal rights risk to persons in care.
