Director also agrees to submit a statement of understanding This requirement is not met as evidence by: Per director, on approximately 12/8/2025 facility had a total of approximately 6 children that were exposed and/or diagnosed with Hand Foot Mouth. 8 of regulation via email LPA.

Director/ Administrators will ensure all individuals subject to criminal records review will have criminal records clearance, transfer clearance, or criminal records exemption prior to working or volunteering in the facility. Per records review, staff S1 begin work at the facility on August 26, 2024. Fingerprint clearance was completed on September 2024.

Based on record review and interview, the licensee did not comply with the section cited above. Staff #1 did not have current mandated reporter training certificate, which poses a potential Health, Safety and, Personal Rights risk to persons in care. 8
