Based on record review, the licensee did not comply with the section cited above in 5 of 9 staff files, Staff #2, 3, 5, 6, & 9, did not have record of immunization for Pertussis, Measles, and Influenza, which poses a potential health, safety or personal rights risk to persons in care.

Per Director, they will complete the Step 2 Orientation, the Mandated Reporter Child Care Provider (AB1207), and Preventative Health and Safety course. LPA provided EMSA website to director. Director will submit proof of completion for Step 2 orientation and Mandated Reporter Based in interview and record review, Licensee did not ensure that the interim director placed in position, met the full requirements of a Director by the 30th consecutive calendar day. This poses a potential risk to the health, safety, and personal rights of children in care. 8 and at least the proof of registration for Preventative Health and Safety course, by POC date. Proof of completion of Health and Safety will be submitted to LPA when done.
Based on interview, licensee did not ensure supervision was maintained at all times at the facility. Child #1 and #2 were found on the back playground by staff, without any adult supervision, which posed and immediate risk to the health, safey, and personal rights of children in care. 8 of what supervision is in the child care center. Director will submit staff meeting agenda, signatures form acknowledging each staff members attendance, and copies of the videos summaries, to LPA by POC date.
