D1 stated that S1's employment was terminated on 3/8/26. D1 understands the importance of ensuring the facility verifies teacher qualifications and maintains accurate records at the facility. Record review of S1's employee file revealed that S1 had 9 of 12 units required to be a fully qualified teacher which poses a potential risk to the health, safety, and personal rights of persons in care.

Based on record review, the facility had 1 drinking fountain in the hallway that exceeded the allowable levels of lead in the water (8.3 ppb). This is a potential health and safety risk to children in care. and children bring their own water and receive drinking water from a filtered water dispenser which was tested.
