Based on LPA observation the licensee did not comply with the section cited above. LPA observed children using the restroom without supervision on three separate occasions during the facility inspection. LPA also observed a child eating at a table unsupervised while teachers were with other children in another area of the classroom. Based on these observations, this poses an immediate health, safety or personal rights risk to persons in care.

Director self reported an incident on 12/15/2022 of a child exiting the playground through a gate and walked onto the front facility parking lot unsupervised. This poses an immediate risk to the health, safety or personal rights of children in care. 8 IN CHILD CARE CENTERS. Director is to submit a signed staff roster as evidence that ALL staff have reviewed the CCL video by 1/13/2023. An immediate $500 Zero Tolerance penalty is being assessed today.
