101700.3 (b)(2): Licensees shall maintain a lead value at ... 5 ppb in all outlets subject to the testing requirements of these Written Directives... This requirement was not met as evidence by: 1 Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Acton level. Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlet B(9.0 ppb) and C30 (5.9 ppb). This is a potential health and safety risk to persons in care. 8 Facility will notify CDSS with completion within 30 days by 9/25/23.
Based on LPA observed bleach bottle accessible to chilren on a shelf adjacent from the children's bathroom. LPA observed a teachers purse and other personal items in a bag on the floor in the classroom accessible to children. LPA observed white out in unlocked teachers desks, bleach bottles in unlocked teachers desks and adult scissors, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care.
Based on carbon monoxide detector did not have a battery, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on director self-reporting the incident of two children left out on the playground without supervision. This is an immediate risk to the Health and Safety of children in care. Civil Penalty LIC421IM assessed. Plan of correction discussed with director
