Based on the interviews and record reviews, it was proven that there were multiple occasions where center staff signed-in and/or signed-out the children which posed a potential health, safety, or personal rights risk to children in care. 8 Facility representative was given an email for submission of the sign-in and sign-out sheets: David.Nguyen@dss.ca.gov Facility representative will submit the LPA copies of the sign-in and sign-out sheets.
Based on the interviews and record reviews, it was proven that child’s records were not given to child’s authorized representative, who had an authorization by law, which posed a potential health, safety, or personal rights risk to children in care. 8 Facility representative was given an email for submission of the sign-in and sign-out sheets: David.Nguyen@dss.ca.gov Facility representative will submit LPA a copy of the plans to maintain child’s records and make child’s records available to child’s parents and/or child’s legal guardians’ inspection if needed.

Based on observations and interviews, the licensee did not comply with the section cited above by leaving the children unattended while the site supervisor was stepping away from the classroom to go to the front door to open the door for LPA, and the other teacher was in the kitchen preparing lunch for the children in which poses an immediate health, safety or personal rights risk to persons in care.

During today's visit LPA observed that the affected water faucet is not in use. Director stated that once she was informed, she took immediate action. The affected faucet will be replaced on 8/12//24, and the water will be retested. The water report dated 06/18/2024 revealed that the facility had elevated levels of lead in one water faucet. This is a potential health and safety risk to the children in care. 8 Director will email LPA Nguyen photographs of the sink once the water faucet is replaced and the lead test result once obtained.
Based on interviews conducted, it was revealed that facility staff grabbed the childcare child’s arm, leaving a grab mark and causing pain. This poses an immediate health, safety, or personal rights risk to children in care. 8 Director, Nadia Khan will email the signed statements to LPA.

(a) Each child receiving services... shall have certain rights that shall not be…abridged...These rights include...the following:…(2) To receive safe, healthful, and comfortable accommodations... This requirement was not met as evidence by: 1 Licensee and her staff will check diapers every two hours and will check the diaper of each child before they are allowed to go home for the day. In addition, Licensee will create a daily diaper log for the children that will track diapering and will be provided to children’s families upon request. Interviews revealed issues relating to diapering, including not being changed throughout the day and picking children up soiled at end of the day. This poses a potential risk to the health and safety of children. 8 Licensee will provide LPA with a copy of this form once it is created.
