Based on LPA's observation and record review, a teacher's aide (S1) alone in Room 10 supervising 10 children. This posed an immediate risk to the health, safety, and personal rights to persons in care. 8 Because the facility repeated the same violation within 12 months, an immediate civil penalty of $1,000 is hereby assessed. An ongoing civil penalty of $100 per day will be assessed until the violation is corrected.
