Based on interviews, on 4/25/2025, a plastic water bottle filled with alcohol was found on a middle shelf in an unlocked cabinet. When it was found it was removed from the classroom, and confirmed by MVPD that it was alcohol. This poses a potential health and safety risk to children in care.

Staff-Infant Ratio. There shall be a ratio of one teacher for every four infants in attendance. This requirement is not being met as evidence by: Although most children were napping, some children were observed to be 1 Facility representative is to create a plan to ensure that proper ratio is maintained at all times. Plan is to include how facility will ensure proper ratio during napping/waking/transition times. Plan to be emailed to LPA no later than 9/18/2024. up and walking around in the classroom. During this time there were 22 infant aged children present with 2 staff members. This has the potential to be a health and safety risk to children in care. 8
