Annex Montessori Childcare Center

Per facility representative, facility will ensure that there are two staff present when children are at the table for any activity. Smaller groups for activities has been set in place to ensure all children are supervised. A staff meeting was held and a meeting agenda will be submitted to LPA. All staff interviewed revealed that C1 fell and hit their head; however, there are discrepancies on how it happened and who witnessed the incident. The reason for the conflicting statements is unknown which poses a potential health, safety or personal rights risk to persons in care.

Based on interviews conducted, the licensee did not comply with the section cited above in that Child # 2,8 &10 disclosed teachers yelling at children. Specifically, child #2 disclosed that teachers yell at child # 1 and child #10 disclosed that staff #3 yells at child #2. 8 a declaration written by each teacher of what they learned to LPA by 04/03/2023.
