Based on interview and record review, the facility did not comply with the section cited above in that on August 12, 2026, there was an absence of supervision resulting in C1 being left unsupervised in a classroomwhich poses an immediate, safety, or personal rights risk to persons in care. 8 Director will submit the staff attendance sign-in sheet, training agenda, training summary, and a supervision and child-transition plan to the SDCCRO no later than 8/21/26. Director also stated that she will attend an in-person meeting at the SDCCRO to discuss supervision and child-transition procedures with the LPM.

Based on record review, the licensee did not comply with the section cited above as staff member #1 (Teacher) did not have at least six postsecondary semester or equivalent quarter units of the required education or a child development assistant or teacher credentialing permit, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as two children (children #1, #2 -listed under the LIC811) ages 18 through 24 months of age enrolled in the toddler component did not have the following sleep documentation on file: date, infant’s name,time of each 15-minute check, initials of staff person who conducted each check, which poses a potential health, safety or personal rights risk to persons in care.
