Based on interviews and record review, C1 and C2 were left unsupervised in the classroom during nap on October 28, 2024. When alone and unsupervised inappropriate contact occurred between C1 and C2. This poses an immediate risk to the health, safety or personal rights of the persons in care.

Based on record review and interviews, C1 a school age child was not separate from the preschool children on 10/28/24. C1 and C2 engaged in inappropriate contact in the preschool classroom, this posed an immediate risk to the health and safety of persons in care. 8 A newsletter will be sent to the parents informing them of the daily drop off procedures and changes in schedule of care. .

Based on observation, the 1 Per Administrator, a staff member immediately placed in the room with C1. A staff training will be conducted regarding supervision. A copy of the sign in sheet for the training will be submitted to CCL. Center will ensure no child is left without supervision again licensee did not ensure C1 was not left without supervision, as C1 was observed alone in the toddler room at 1:45pm and 2:40pm which poses an immediate health and safety risk to children in care. Civil penalty of $500 was assessed.
