This requirement was not met as evidenced... Licensee will develope and submit a written plan describing how the facility will ensure that all staff interact with children using positive, and developmentally appropriate language. Plan must include steps to prevent violation of Personal Rights 101223(a)(1). Based on interviews and video obtained, the licensee yelled in the presence of children. And yelled directly at a child during nap time in the presence of children and teachers. Which poses an immediate health, safety or personal rights risk to persons in care. 8 Licensee will develope and submit a written plan within 24 hours describing how steps to prevent violation of Personal Rights 101223(a)(1). This requirement has not met as evidenced: Staff #1 forcefully took a cup away from Child #1 while the child was still drinking, and 1 Facility is required to submit a Plan of Action and Correction which will address Staff #1 interaction with children. Staff #1 took the child's cup, they forcefully grabbed and pulled Child #1's arm to join the group. Staff #1 said loudly and aggressively "The blanket is mine until you are settled down!", "Stop!", "Be quiet!" while the children were settling down on cots.This is an immediate risk to the health and safery of children in care. 8

This requirement has not met as evidenced: part of the fencing not being fully submerged into the ground leaving sharp wires protruding, and 1 Facility Representative stated the maintence crew will come out today and proof of work order will be submitted to LPA Maryanova to Julia.Maryanova@dss.ca.gov fence sections in a Junior Pre-School age component that have been repaired with wiring, some of the wiring is sharp to touch. This is a potential risk to the health and safety of children. 8

101229(a)(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). This requirement was not met as evidenced 1 Director stated the name to face document used by staff during transitiona has been updated for clarity and accuracy. Director also stated the two staff present at that time have received additional supervision and training. Director stated she has arranged for a third staff to join the two teachers at that by multiple witness statements that a child (Child1) was left unattended on the playground for one to three minutes. This incident was self-reported by the facility. This poses an immediate health and safety risk to children in care. 8 last daily transition to ensure name to face checks are accurate and that children are all accounted for and under supervisoin with no errors.
