interviews and photos reviewed of: 1 Staff will be hired to cover for the awake and napping infants without placing awake infants in the nap room in a crib while awake. Awake infants being placed in the nap room in a crib to play while napping infants are in the nap room sleeping which posses an immediate danger to the health and safety of children in care.
interviews and observations by LPAs when an aide(S1) was left in the infant room feeding 4 infants with no fully qualified teacher 1 Fully qualified staff will be hired to cover all shifts when children are attending. which posses an immediate risk to the health and safety of children in care.
observations of LPAs 1 Lead teacher, Angie, will discuss with owner Paolo regarding spraying and pest control. Receipts will be kept to supply to LPA Lea Habtom to clear deficiency. Owner was not present during inspection. of dead cockroaches on the floor and interviews which revealed that the facility has had cockroachs in the infant play area, cribs and office space which posses a potential danger to the health and safety of children in care.
LPAs Blesi and Habtom observed Child #1 swaddled while in care. If not corrected, this is an immediate risk to the health and safety of children in care. 1 LPAs discussed safe sleep practices with staff and staff immediately removed the swaddle from child #1. Deficiency corrected during inspection today.
Notice of Site Visit Posting Requirement: The notice posted pursuant to subdivision (a) shall remain posted for 30 consecutive days, except that a family day care home shall comply with the posting requirements contained in this subdivision only during the hours when clients are present. LPA provided another notice of site visit dated 12/20/22 and LPAs observed director post it at the end of the inspection. (2) Failure by a licensed child day care facility or a family day care home to comply with paragraph (1) shall result in an immediate civil penalty of one hundred dollars ($100). This requirement was not met when LPAs Blesi and Habtom observed facility failed to post the notice of site visit with Type A and Type B deficiencies from inspection dated 12/20/22. Civil Penalties are assessed in the amount of $100. 8
The director failed to provide qualified staff on the morning of 10/18/2022 when an aid and a volunteer (trainee) were left to supervise infants until a qualified staff (Ms. Angie) arrived. This poses an immediate risk to the health and safety of children in care if not corrected.
There shall be a ratio of one teacher for every four infants in attendance. This requirement was not met as evidenced: on 10/18/2022, LPA Blesi observed a teacher, and a trainee supervising 7 infants. A trainee may not be used in ratio; therefore, the facility was out of ratio at that time. This poses an immediate risk to the health and safety of children in care if not corrected. 1 Director states they have hired additional qualified staff and they are following ratio requirements during today's inspection. LPA will conduct a follow up inspection to verify ratios are being followed.
Paolo states the employee was immediately given a formal written reprimand. All staff were then instructed to obtain head counts of the children before and after going to the playground. Child #1 was left on the playground without supervision for a period of time. This is an immediate risk to the health and safety of children in care. Civil Penalties are assessed in the amount of $500.
Interviews revealed parents did not receive a copy of the Type A citations issued 3/15/22. In addition, a newly enrolled parent did not receive a copy of the Type A citations and there was no documentation (LIC9224) retained in children’s files per a file review conducted on 10/18/22. This poses a potential risk to the health and safety of children in care.
Diector states the school age children were going over to the infant classroom because their mom is a teacher there. He states he will no longer allow children over age two to be in the infant classroom. LPA will make a return inspection to verify correction has been made. The facility failed to follow the age specified on the infant license when they allowed two school age children ages 7 and 8 years old in the infant room in the morning which poses a potential risk to the health and safety of children in care.
Based on observation the licensee did not comply with the section cited above in LPAs observed cleaning supplies ( bleach, Windex, powdered scrub) under the kitchen sink and the bathroom sink. The cabinets were not secured with a child safety latch making the items accessible to children. This poses an immediate health, safety or personal rights risk to persons in care.
Based on observation, LPAs observed a staff leave the infant napping room for about 1 minute. This occurred two times at 2:15pm and again at 2:22pm which poses an immediate health, safety or personal rights risk to persons in care.
Based on observation LPAs observed an aide S1 left alone supervising a group of 5 infants which poses an immediate health, safety or personal rights risk to persons in care.

a staff who was supervising two infants eating in high chairs stepped around the corner to go into the kitchen area leaving the two infants in high chairs without direct visual observation and supervision. This is an immediate risk to the health and safety of children in care. 1 Paolo states that he will have converstation with staff regarding 100% visiual observation and supervision of infants as well as create a new staff schedule to ensure there are always sufficient staff supervising the infants so if one staff needs to step around the corner or change a diaper there will be sufficient coverage. To correct the deficiency owner shall submit a revised staff schedule to LPA.
