Based on record review,two out of two files did not have current Mandated Reporter Training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, one out of two files did not have a copy of immunization record: MMR and tdap, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on one out of two files did not have a completed Health Screening, LIC 503, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA observation of record review, video footage, interviews children, who disclosed that an incident happened where Child#3 hit Child#1 on the forehead with his foot, while was in care on 06/29/2022, which affect the physical well-being of the child#1. Which poses immediate Health, Safety or Personal Rights risks to children in care. training agenda along with the Staff attendee signature via email no later than 10/17/2022. Additionally, Director will submit to LPA via email copies of the form LiC9224 Acknowledgment of Receipt of Licensing Reports sign by parent for all children enrolled no later than 10/12/2022.

on 7/28/22 Based a review of Guardian showing that Teacher Assistant Ms. Phoebe Fischer was not associated to the facility. 1 Staff#2 shall take live scan Staff will be able to return, after Director confirm that Staff#2 has been associated to Barefoot Preschool. Ms. Fischer was not present during this inspection, however Teacher was present today, but was working from the period of 6/22 to 7/12/2022 while Director was on vacation. This poses an immediately H&S to children in care. 8 LPA provided lic9224 form to Director.

on 7/28/22 Based a review of Guardian showing that Teacher Ramirez was not transfered/ associated to the facility. however Teacher was present today, but was working from while Director was on vacation. This poses an immediately H&S to children in care. 8 Staff will be able to return, after Director confirm that Staff#1 has been associated to Barefoot Preschool.

on 7/28/2022 Director provided a declaration, how she will maintain her facility all time with Teacher qualified. At 8:20am, Upon arrival, LPA observed staff#4 alone with 4 children in care. Director was not present. During file review, it was found that Staff#3 was not fully qualified teacher. This poses an immediately H&S to children in care.

on 7/28/2022 1 Director agreed to provide via mail or drop off at El Segundo no later than 8/5/2022 to LPA Miranda based a file review and interview, Director was unable to produce the complete file for Staf#1, #2, #3 and #4. This poses a potential H&S to Children in care.
