Based on interviews and documents recieved it was found that staff will tell children that they will take videos or photos of children and send to their parents if they do not listen. This poses a potential health and safety risk to children in care.

Based on interviews and review of supportive documents it was found that there were instances where staff inappropriately handled and inappropriately spoke to children in care. This poses a potential health and safety risk to children in care.

Based on observations, interviews, and record reviews there were 55 children present when the facility has a listed capacity of 50 children. This poses an immediate health and safety risk to children in care.

Based on observations 9 children entered restrooms without adult supervision and were left unspervised for approximated two minutes until S2 found them. This posed a potential health and safety risk to children in care.
