Bay Area Child Care - Millard Preschool

Based on interview & record review, the licensee did not comply with the section cited above in 1 out of 3 persons present at the facility which poses an immediate health, safety or personal rights risk to persons in care. S1 was found to have no criminal record/fingerprint clearance.

Based on observation, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. During the inspection around 9am, LPA Uribe saw one staff member walk into the office while the second staff member was already inside of the office. This left the 4 children present at the time withuot direct visual supervision. LPA immedaitely asked one of the staff members to please step out of the office to supervise the children. This happened twice within a 30-minute time span.

Based on observation the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. An aide was observed taking two children to their classes on campus at 9:30am. When the aide returned, LPA Uribe noticed that she was not in the presence of a teacher qualified staff member and therefore was not in compliance. Aides are never to be left alone with the children in care.

Based on record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. The two children enrolled in the Preschool did not have two required forms in their file. The forms missing are the Identification & Emergency form (LIC 700) & the Consent for Emergency Medical Treatment form (LIC 627).
