Staff briefly operating out of ratio due to a staff shortage related to the sudden passing of a staff member, main director’s medical leave of absence, and initial interim director's leave of absence for travel. 8 Ensure adequate staffing by providing additional substitute teachers, hiring new teachers, and anticipating staffing shortages. Provide a guideline to LPA for the above mentioned.

Interviews disclosed that evidence of rodents were found inside the facility premises which poses a potential risk to the health and safety of the children in care. The facility has contracted a new pest control company to better serve the facility needs to prevent pest infestation. The facility is conducting daily morning inspections.

Based on observation, and tour of the facility, the office where the staff and children's files are stored was left open on 9/15/25, and the drawers of the file cabinet where the files were stored were not secure which poses a potential health and safety risk to children in care. 8 Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. 1 Director agreed to send record of completion to LPA Via email by Friday 6/21/24 Based on observation and interview. Staff and parents have stated there was construction that was going on during the hours of operation. LPA observed cones on the roof of the building indicating that work was being done on the roof. 8
