This requirement was not met as evidence by… 1 Director stated she would hold a staff meeting reviewing personal rights regulations, writing ouch/behavior reports for parents, policy for staff to report to Director, keeping toddlers & preschoolers separate – and have all staff sign meeting agenda and provide this to LPA Lane by 3/25/22 via e-mail: Keturah.Lane@dss.ca.gov Based upon photos/documents received and interviews with staff members and enrolled children, the facility is not doing enough to prevent a child in care from biting other children which is an immediate health, safety and personal rights risk to children in care. 8

… 1 Director stated she will provide a written statement by 3/25/22 explaining how she will keep the toddler children separate from both infant and preschool areas including areas of facility, staffing needed and how this information will be shared with all staff working at the facility….and e-mail to LPA Lane at: Keturah.Lane@dss.ca.gov Based upon staff and Director interviews, a toddler child was co-mingling with preschool age children at beginning and end of workdays when enrollment was low and child was biting other children in care as a result which is an immediate health, safety and personal rights risk to children in care.

… 1 Director will provide proof of fingerprint clearance and association to facility by 3/25/22 via e-mail to: Keturah.Lane@dss.ca.gov Based upon LPA observation and record review, volunteer Nancy Wright was not fingerprint cleared or associated to the facility which is an immediate health, safety and personal rights risk to children in care.

… Director will provide written plan stating how she will ensure sign in and out sheets are accurate and signed by the authorized individual and provide this by via e-mail to: Keturah.Lane@dss.ca.gov Based upon documents received from other sources and interviews with parents and outside agencies, some of the sign in and out sheets were not completed by the person bringing/removing the child from the center which is a potential health, safety and personal rights risk to children in care.
