(a) The licensee shall provide care and supervision ..(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met evidenced by: 1 Per director, facility performed correction action with teacher, reinforced name to face check in procedures with all staff, and admin is conducting daily checks of name to face log. All documents were provided to LPA during visit. Based on interview and record review, the licensee did not comply with the section cited above as child walked away from teachers unnoticed during transition and was unsupervised for some minutes before being returned to the classroom, which poses/posed a potential health, safety or personal rights risk to persons in care. 8
