Personal Rights-(a) The licensee shall ensure that each child is accorded the following personal rights: (1)To be accorded dignity in his/her personal relationships with staff and other persons. 1 Early Learning Program Administrator stated that she will submit the written plan by 5/11/26 that children's personal rights are not violated. This requirement was not met as evidence by: Staff cut a child's hair without permission from parents. This poses a potential risk to the health, safety and personal rights to children in care. 8

This poses a potential risk to the health, safety, or personal rights of children in care. 8 Staff continue to serve the Children(Room 25) filter water and refilling their individual water container. Children have access to water.

Lead Testing Written Directives - a result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. The fountains are covered with plastic and currently not being used. The children are bringing their own water bottles and they have access to a refill water station if needed. Based on record review, the Lead Sampling Report in three identified Drinking Fountain/ Bubbler in Room 25, 27,and 30 had elevated lead above 5.5 ppb. This poses a potential risk to the health, safety, or personal rights of children in care. 8
