(b) There shall be a ratio of one teacher for every four infants in attendance. This requirement was not met evidenced by: Per director, class sizes were reduced, extra teachers were provided for support during breaks, transitions, and one on ones. Re-training was provided for all staff, disciplanary actions were also taken towards staff. Based on observations, interviews, and record reviews, one staff was stepping away from children and co-teacher leaving co-teacher out of ratio, which poses a risk to the health and safety of children in care. 8 Ratio issues were corrected by MAY. Training sing in sheets, agendas, and reports were all provided to LPA S Li.

Personal Rights
(a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. This requirement was not met evidenced by: 1 Per director, re-directing, modeling, and coaching the teacher through out the school day was implemented. Resources and training was provided consistently. Disciplinary action was taken towards employee. Based on observations, interviews, and record reviews, one staff was reported to speak harshly to older infants, which poses a risk to the health and safety of children in care. 8 Employee is no longer working at facility. Disciplinary reports and training agendas were provided to LPA S Li.
