The licensee shall comply with all terms and conditions set forth in the admission agreement. Facility Representative states will amend and submit to LPA admission agreement to outline instances where less than 2 week notice can be given no later than close of business 11/7/23. Staff also states will in future ensure that facility is following admission agreement terms. Based on information obtained during interviews and record review Licensee failed to comply with the regulation above as child 1 and child 2 were terminated in violation of admission agreement which posed a potential health, safety or personal right risk to children in care. 8

... Licensee will remove fence and old equipment permanently by 11/22/21. Licensee will send pics of corrections to LPA Lane via e-mail. Keturah.Lane@dss.ca.gov Children are not to use "big playground" until hazards have been fixed. Based upon LPA observation, the fence has an area that has sharp metal & accessible to children as well as a gap between the wall and fence where a child could squeeze by. The other side of the playground not in use is not fully enclosed by a fence and potenetially accessible to children. This is a potential health, safety and personal rights risk to children in care.
