The facility did not ensure that 1 Per Director stated the she will clean the preschool classroom and bring back the preschooler into their licensed classroom. A follow up visit will be conducted. the infant classroom operated within the limitations of the license. The toddler classroom had 5 preschoolers and 1 toddler in the toddler classroom which is licensed under the infant. This poses a immediate safety risk to children in care.
Based on observation and interview, Licensee did not ensure that the infant classroom is kept in ratio at all times. LPAs observed 5 infants with one staff during today's inspection. This poses an immediate risk to the safety of children in care.
Based on observation and interview, Staff 5 did not ensure that Child 4 was removed from the infant rocker in an appropriate manner by tilting the rocker forward causing Child 4 to fall out of it. This is an immediate risk to the personal rights of children in care.
Based on record review and interview, Licensee did not ensure that Staff 1 met the educational requirements prior to hiring them as an assistant in the facility. LPAs observed Staff 1 providing care in the toddler classroom. This poses a potential threat to the personal rights of children in care.
Based on observation Licensee did not ensure that the infant classroom operated within the limitations of the license. On 3/26/24, LPAs observed Child1 who is over 2 years old, in the infant classroom. This poses a potential safety risk to children in care.
Based on observation and interview, the Licensee did not ensure that the infant napping space is physically separate from the indoor activity space. This poses a potential risk to the health, safety and personal rights of children in care.
Based on interview and observation, Licensee did not ensure that Child 3 was provided appropriate napping equipment. Per Staff 5, Child 3 uses a big pillow in the classroom to nap. This is a potential risk to the health, safety and personal rights of children in care.

Per the Director on 10/13/22 an Unusual Incident occurred at the facility in which a vehicle crashed into the fence surrounding the Infant outdoor play yard while children were inside the classrooms and the Director did not report the incident to CCL within the allotted time which poses/posed a potential health, safety or personal rights risk to persons in care.
