Facility will create an outdoor supervision plan and provide to LPA. Facility will ensure that when children are outside on the preschool yard they will go out in smaller groups, half the number of total children for that day will go outside at a time. Supervision shall include visual observation. This requirement is not meet evidence by: No staff saw Child #1’s incident, in which child #1 sustained an injury requiring medical attention in which child sustained a 1 cm laceration to the upper lip involving the vermilion border, which required three sutures, which poses an immidtae risk to the health and safety of the children in care. 8

(1) Nochild(ren) shall be left without the supervisionof a teacher at any time, except as specifiedin Sections ... 1 Per facility director, Cynthia, a meeting was held with staff on 3/5/2026 and reviewed transition and name to face procedures. Classrooms will transition in whole groups with two staff when possiable to help ensure the transition is completed with sweeps and leaership team will check on transitions reguarly to ensure they are being completed properly. Supervision shall include visual observation. This requirement is not meet evidence by: LPA Perry determined that Child #1 was left unsupervized for approximately 34 minutes. 8 Additionally, The director will conduct daily supervision checks every 30 minutes and monitoring of classroom transitions for the next 30 days to ensure staff are following required supervision procedures via a form and send to LPA Perry daily by the end of the day. Director did send out an email to all staff on friday 2/20/2026 remininding all staff to maintain accurate attendance records. Supervision shall include visual observation.
