Based on observation and record review, the licensee did not comply with the section cited above During file reviews and interview it was revealed that S1 is the only adult present at the time of them typically transporting children from a nearby school to the child care center. However during file reviews it was discovered S1 does not have a current up to date first aid/CPR including pediatrics. S1 obtained a Basic Life Support certification which poses a potential health, safety or personal rights risk to persons in care.
Based on interview, the licensee did not comply with the section cited above in not ensuring a daycare child was picked up from their school at their scheduled release time, which posed a potential health, safety or personal rights risk to persons in care.
Based on observation and record review facility was commingling the children in care on 09/19/2024 in the Toddler classroom and the school age classroom. >>> 8 >>>There were 3 preschoolers under the age of five present with school age children in the school age classroom and 3 preschool children over the age of 36 months present in the Toddler classroom.
Based on observation and interview, the licensee did not comply with the section cited above ithe school age classroom had a bathroom with dirty toilet fixture. Director stated the rooms are cleaned at end of day each day once the children are gone. which poses a potential health, safety or personal rights risk to persons in care.
Based on observation and interview, the licensee did not comply with the section cited above in he outside playground was full of cobwebs and debris, photos were taken, director stated the playground is cleaned daily. This poses a potential health, safety or personal rights risk to persons in care.
Based on observation, interviews and record review, commingling occurs with school age children are combined in building two in the first hour of the day and then in building one for the last hour and a half of the day. Ranging in ages of thirteen (13) months to nine (9) years old.
Based on observation, interviews and record review, school age children are being combined in the first and last hour of the day, with children as young as 13 months with only one teacher and no aide. Due to school agers being commingled ratios are being exceeded.
