The facility shall submit a written plan to the Community Care Licensing Division (CCLD) outlining how staff will consistently ensure visual supervision of all children in care. All staff shall undergo mandatory training on Title 22 supervision requirements, with emphasis on always maintaining direct visual supervision, including during nap periods. The facility shall obtain staff signatures to confirm their understanding of the training. Additionally, all staff will be required to view the "Supervising Children in Child Care Centers" video available at ccld.childcarevideos.org and submit a written statement summarizing their understanding of proper supervision practices. These statements will be collected and submitted to CCLD by the Plan of Correction (POC) date. Based on interviews, and record reviews, the Licensee did not comply with the section cited above, as of April 15, 2025, the lack of visual supervision resulted in a delayed response to a medical emergency, which poses an immediate risk to the health, safety, and personal rights of the children in care.

Based on interviews and record reviews, the Licensee did not comply with the section cited above. Staff member S1 was observed supervising a group of 14 children during the scheduled nap period. Out of the 14 children, two were awake and active, which placed the teacher out of ratio. This staffing arrangement poses a potential risk to the health, safety, and personal rights of the children in care. 8 The regional office will be conducting a follow up meeting with this facility to discuss all deficiencies.
