Based on observation, interviews and record review, the facility did not comply with the section cited above that the facility stored the bread which lapsed the date “best by” in the freezer to be planned for serving to the children in care. There was insufficient food to be served at lunch for the children in care, which poses/posed a potential health, safety, or personal rights risk to persons in care.

Director agrees bring outdoor water dispenser for all children outside during play. Director states that this will begin 9/15/21. Director agrees to send LPA a picture of the new dispenser in use. During the inspection, LPA observed that children in the play yard did not drinking water available. This poses a possible health and safety risk to children in care.

Director agrees to replace shade in children's play area by 9/17/21 and send LPA a picture of the replacement. During inspection LPA observed that the shaded area in play yard had been removed. Director states that it was removed 2 months ago. This poses a possible risk to children in care.
