Licensing Program Analyst (LPA), Jaime Gonzales spoke with Licensee Manali Doshi on the phone and reviewed the regulation. Licensee will provide a statement via email for future constructionor alternation of buildings. This requirement was not met as evidence by: Licensee did not inform the department of the alternation of infant playground prior to the construction occuring. 8
