Based on record review, observation and interview, the facility did not ensure that the center was operating within the conditions & limitations specified 1 LPAs discussed issue of operating beyond the scope of the license with Director. LPAs observed Child 1, Child 2 and Child 3 be picked up from the facility. Director will submit documentation confirming termination of enrollment for Child 1, 2 and 3. on the license since there were 3 infant children and 1 school age child present during today's visit and Child 1 who is an infant had been present in the preschool room during previous visits by the Department. This is an immediate risk to the health and safety of children in care. 8 Director will submit a declaration stating understanding that neither infants, children under 2 years old nor school age children can be provided care in the preschool program. A non-compliance conference has been scheduled with the Department.
Based on 1 Director has submitted a birth certificate for child 1 confirming they are under 2 years old. Upon record review, Staff 2 observed providing care to infant children has been enrolled in classes to comply with education requirements. observation, interview and record review the Director accepted & submitted forms that had an altered DOB for Child 1. Director allowed infants to be provided care by unqualified staff although an infant license has not been approved. This is an immediate risk to the health &safety of children in care. 8 Director will submit a declaration stating parents will be required to submit a child's birth certificate upon enrollment going forward and also stating that all staff providing care at any time will be qualified. A non-compliance conference has been scheduled with the Department.
