LPA observed a variety of toys in outdoor play area not maintained such as plastic play equipment to be cracked and have broken pieces exposing sharp edges, sensory table arch way leaning with bent metal bar, and bicycles and scooters to be missing handle bars and rusted at time of inspection. This is a potential health, safety, and personal rights risk to children in care.
Based on record review and interview, the Licensee did not comply with the section cited above in 5 out of 6 staff files not having proof of AB1207 Mandated Reporter training completion which poses/posed a potential health, safety or personal rights risk to persons in care.
