Based on interview and record review, the facility did not comply with the section cited above. A child was left unattended and unsupervised, posing a potential health, safety, or personal rights risk to children in care. 8 The staff member involved with the incident has been terminated. The deficiency will be cleared during today's visit.

Based on record review, LPA Leong determined that S1 submitted an edited Mandated Reporter Training (MRT) certificate, which poses a potential health, safety or personal rights risk to children in care. 8 The plan must clearly describe the steps the facility will implement to ensure all future documentation submitted to Licensing is accurate, current, and unaltered. The POC is due on July 15, 2026.

A staff member handled C1 roughly and inappropriately on two occasions, which did not align with safe, supportive, or developmentally appropriate practices. This action posed an immediate health and safety risk to the children in care.
