(a) The licensee shall provide care and supervision..(1)No child(ren) shall be left without the supervision of a teacher at any time. This requirement was not met as evidence by: 1 I will schedule a meeting with the family no later than August 14, 2026 and I will assign training to the staff as well and provide proof of the training to Licensing by the POC date. Based on interviews and obervation C1 ran out of the classroom and staff chased behind but was not aware C1 left the facility. A different staff saw C1 outside in the front parking lot area and brought C1 back inside the facility this is a potential health, safety, or personal rights risk to the persons in care. 8

Based on interviews and observation two children C1 and C2 were allowed to leave the classroom undetected by staff and were alone on the play ground without staff supervision for sixty four seconds which is a potential health, safety, and personal rights risk of the persons in care.

Based on interviews and record, it was determined that C1 was left unsupervised in the breezeway of the facility for approximately three minutes. This poses a potential risk to the health, safety or personal rights of the children in care.
