Based on interview and record review, the licensee did not comply with the section cited above in 6 out of 9 staff sample files had expired Mandated Reporter Certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on witness statements and record 1 Director stated she has an additional staff go into that classroom during transitions, increased staff hours to ensure overlapping coverage, provided retraining on the transition process and staff assessment of children's likelhood to need more guidance. Director stated she will email her written plan reviews, the facility did not comply with the section cited above as, in a self-reported incident, a child was left unsupervised on the playground for 40 seconds. Based on witness statements and record 1 Facility representative stated she has already done a best practices review and review of name to face procedures, and will develop a detailed written plan for ensuring there is not another incident of absence of supervision. The facility self-reported that Child 1 was left in a classroom bathroom without staff supervision for two minutes. This is a potential health, safety, and/or personal rights risk to children in care. 8 Director statyed they will provide to LPA a copy of the agenda and sign-in sheet for staff who attended/completed the training. LPA observed in six out of eight classrooms 1 Assistant Director shall make a staff schedule indicating that the classroom has at least two teachers until all of the children are asleep. Assistant Director shall submit the schedule to LPA via email by COB on 7/23/22. during nap time, there was one teacher alone with a minimum of 18 children while multiple children were still awake, using the bathroom and trying to get settled. This is an immediate health and safety risk to children in care.
