Based on observation, interview, and record review, the facility did not comply with the section cited above during the staff breaks as there are two fully qualified teachers and one aide with no ECE units present supervising 25 children. During today's visit, LPA also observed 2 qualified teachers with 25 children in the outdoor area, while the aide is inside the facility prepping and cleaning. This poses a potential health, safety or personal rights risk to persons in care. 8 Additionally, the director states that the owner will also try to step in and LPA also sugested going through a staffing agency. Documentation will send proof by set due dates.
Based on observation, interview and record review, S3 (whom was present during LPA's visit), did not have fingerprint clearance prior to working with the children on site, which poses an immediate health, safety or personal rights risk to persons in care.
