Infant Care Center Sleeping Equipment. Cribs are to be free from all loose articles, including blankets and pillows. This requirement was not met as evidence by LPA's observation of five sleeping infants in cribs with blankets and toys. 1 Per Program Director, Brittney Randolph, training will be provided to infant staff. A copy of detailed training agenda and attendance will be sent to Community Care Licensing Office no later than 4/3/2023. This is a potential pesrsonal rights, health and safety risk to infants in care. 8

Based on LPA's staff record review, the licensee did not comply with the section cited above. Staff #2 and #5 are missing the required Health Exam form that includes physician's comments if staff can meet their job duties. Based on LPA's record review, the licensee did not comply with the section cited above. LPA reviewed six infant files and observed all six files did not have the updated Needs and Services Plan. This poses/posed a potential health, safety or personal rights risk to persons in care.
