Based on interviews, the facility did not comply with the section cited above due to a lack of supervision which poses an immediate health, safety or personal rights risk to persons in care.

the following: 1 Regional Director Nicole Rowe and Cesar Chavez Center Manager Andrea Nolasco had a meeting with T1 on 8/23/22. The meeting and incident were documented and required T1'’s signed acknowledgment of her failure to uphold Catalyst performance/supervision standards. A copy of the Center staff self-reported to Licensing that on August 18, 2022, a lack of supervision lead to a child being left unattended in the classroom. This poses and immediate risk to the health and safety of children in care. 8 acknowledgment was provided to Licensing.
