There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance. This has posed an immediate risk to the health, safety, and personal rights of the children in care. 1 The Director has since hired three additional staff members to assist in ensuring ration remains in compliance. The Director shall submit a letter of understanding explaining how this will be avoided in the future, as well as a copy of the staff schedule showing full coverage at all times by the POC date. All staff interviewed confirmed that the center has operated out of ratio in the mornings, afternoons, and during some traditions. 8 AB633 to issued to all parents of children in care, signed, filed in each child's file, and provider to do the same for all new clients for the next 12 months to this day.

Lead in drinking water §101700.3(b)(1)
During today's inspection LPA Macias requested and obtained the following documents; Self-Certification LIC9275, Sampling Checklist Form LIC9276, Facility Sketch LIC 999, fully labeled with locations of all water outlets, and full lead report. This exceedance level found in the kitchen sink (faucet B & C), levels were 14, and 8.2. This poses a potential risk to the Health, Safety, or Personal Rights of children in care. 8 According to the water report all water usage at fixtures in question must cease. Fixtures must be flushed for 30 seconds, 4 times a day for 3 weeks. The fixtures have all been replaced, and will be tested again in three weeks. Director shall submit a plan on how the facility will move forward by the POC date.

The Director has since conducted an all staff training on care and supervision. The Employee involved in the incident is no longer employed by Catalyst Kids. The LPA obtained proof of the training. The Director shall submit a letter of understanding by the POC date, outlining how this will be avoided in the future. As a result of lack of supervision a chid sustained an injury, the child fractured their femur on the right leg, as well a hip dislocation. This poses an immediate risk to the health, safety, and personal rights of the children in care. 8 AB 633 states upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months and it is to be signed and kept in each child’s file. The Director shall review the regulation and submit a letter of understanding, as well as creating a procedure/ checklist on when to call for emergency medical treatment. The documents shall be submitted by the POC date. After sustaining an injury the child remained at the facility and did not receive medical attention. This poses an immediate risk to the health, safety, and personal rights of the children in care. 8 AB 633 states upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months and it is to be signed and kept in each child’s file.
