Based on interviews C1 sustained an injury while in care at the facility. Staff were unable to recall the events of how C1 sustained the injury, therefore C1's legal guardian was not notified by the facility. This poses a health and safety risk to children in care. 8

Director will be creating an action plan and will provide to LPA. Based on interviews Children were engged in a physical altercaion that was witnessed by S1 bu was not prevented or stopped when noticed.
