101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time.... Supervision shall include visual observation. 1 The director agrees that an effective head count procedure training should be provided to all staff. The director stated that the training agenda and role sheet will be completed and emailed to LPA via email to stella.gutierrez@dss.ca.gov by 02/03/2023 before 6:00 PM. This regulation was not met as evidence by interviews conducted that child #1 was left outside for at least 10-15 seconds without a staff present. Based on the circumstances of time left and distance a type B deficiency it being cited. This is a potential risk to the children in care. 8

Building and Grounds (a) The Child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement not met as evidence by: 1 Facility staff followed Written Directives in PIN 21-21 CCP for lead water testing. Licensing Forms completed; 9275, 9276 and facility sketch submitted. Notification sent out to parents, water outlet flushed (not in use), conditioning for 3 weeks, water filter installed onto pipping, Based on lab report analysis water from faucet B,D, E, F. These faucets samples taken on 05/06/2022 indicate levels of lead in exceedance. 8 PASS sinks readily available for the staff and children. Director stated that she will submit new restest results to LPA
