Based on Staff#1 interview and facility camera 5 video footage, child#1 was left unattended and alone in Room 105 bathroom from 2:26:56 p.m. to 2:35:46 p.m. for 9 minutes. This requirement was not met as evidence by... 1 Per Program Supervisor, she will ensure to communicate with school principal for future event, preschoolers given enough time for transition. Also, staff count children more often during each transition. A written declaration will be submitted to LPA by 03/03/25. Based on staff#7 interview, in November 2024, the day that Rams visited school, child#1 was missing from line to come back to class and child#1 was found in the auditorium. This poses an immediate health, safety and personal right risk to the children in care. 8 $250 Civil Penalty was assessed for repetitive violation within 12 months.

A meeting will be held with Program Director to set a plan of correction. Staff Attendance sheet will be submitted to LPA via email. Based on LPA’s observations and interviews which were conducted with four staff, child#1 was unsupervised and wondered away at school age enclosed yard alone on 01/31/25 from 4:18:34 PM to 4:22:25 PM. This poses an immediate health, safety and personal right risk to the children in care. A written declaration will be submitted to LPA by 02/21/25. $250 Civil Penalty was assessed for repetitive citation within 12 months.

Based on LPA's observation during tour of Room 101 the licensee did not comply with the section cited above and LPA observed approximately at 10AM, when couple children were getting ready to have AM snack, child#2, grabbed its own snack from snack basket which was accessible to the children and spilled it on the carpet by the wall which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on four staff interviews, On 03/18/24, Child#1 left the classroom 105 and left the school anytime between 8:15 AM to 8:25/8:30 AM and was found 33-34 foot steps away from the school. This poses an immediate health, safety and personal right risk to the children in care. 8 $500 Civil Penalty was accessed.
