Based on interviews child (C1) was usupervised for approximately 5-6 minutes on the playground. An immediate civil penalty of $500 applies. This regulation was not met as evidence by: 1 Facility shall hold an all-staff meeting to review licening reporting requirements. This regulation was not met as evidence by: The staff responsible for these violations has been terminated and no longer works at the facility. Licensee to conduct an all-staff meeting to review and discuss personal rights regulations and ways to communicate with children in a positive way. Absence of supervision, including, but not limited to, a child left unattended, and supervision of a child by a person under 18 years of age. 1 Facility immediately placed mesh over concrete wall to inhibit the ability to climb the wall. Facility held an all staff meeting to go over active supervision and safety protocols. Agenda and sign in form was provided to CCL on 10/25/23. This requirement was not met as evidence by: C1 eloped from the facility for approximately 5 minutes and was found down the street. This poses an immediate risk to the health and safety of children in care. 8

Based on interviews and record review the licensee failed to ensure that the facility had the proper information for C1’s medication prior to issuing it to the child since it was not in original container. This poses a potential health, safety or personal rights risk to persons in care. Director agrees to send in a updated medication policy including steps taken when medication is brought to the facility to ensure accountability and proper handling. Additional training and documents will be provided to CCL by March 1, 2022.
