Based on the LPA's interview with the center director on 04/28/23, the center director stated that the facility did not obtain signed LIC9224 “Acknowledgement of Receipt of Licensing Reports” from parents and guardians of each child receiving services in this facility. The facility was cited a Type A violation on 01/04/23. This poses a potential health and safety risk to children in care.

Based on interviews with facility staff on 04/28/23, it was corroborated that due to a lack of supervision, child C1 cut their hair with a pair of children's scissors found in the classroom; this was not observed by any staff member. This poses a potential health and safety risk to children in care. The center director stated that the scissors in the children's classrooms have been made inaccessible to children in care. The center director stated that an all staff meeting will be held on 08/10/23 and cover the topic of care and supervsion, classroom management, and facility policies and procedures. The center director stated that the meeting agenda along with the staff attendance sign in sheet will be sent to the LPA by 08/14/23.

Implemented staff training regarding supervision, will also utilize whiteboards with a head count on the doors. Facility Director will submit plan of correction to CCLD by 6/30/23 This was not met as evidence by: based on interviews and record review, C1 was unsupervised and left the classroom and exited the building. This poses an immediate health, safety, or personal rights risk to persons in care. 8

Supervision shall include visual observation. 1 T1 was dismissed immediately. Camera’s were installed at the front door of the preschool with monitors in the Main classrooms for monitoring of the front door. Alarms for all doors in the preschool were installed by January 6, 2023 This requirement was not met as evidence by: Unusual incident report and interviews, licensee did not comply with the above requirement which poses an immediate health, safety, or personal rights risk to children in care. 8 Additional meetings and trainings were conducted to review protocols and procedures to ensure the safety of children.
