Based on observation, 4 children were sitting outside the classroom door having snack without visual supervision from the teacher, who was inside classroom. This is an immediate health and risk to children in care. 8 ***Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on observation, the bathtub in outdoor play area at bottom of hill had about 6-8 inches of standing water inside, which poses potential health, safety or personal rights risk to persons in care.

Based on observation and record review, child C6's Epinephrine expired 9/21/23, which poses a potential health, safety or personal rights risk to persons in care.

Based on interviews and records review, EMSA approved CPR/First Aid cards were issued to staff without the required training. Former director advised staff that training was not needed due to COVID. This is a potential health and safety risk to children in care. 8 LPA may return on a later date to ensure compliance. Failure to correct will result in a $100 per day civil penalty until corrected.
