Based on LPA’s records review, it was determined that during the period of 03/14/22-03/25/22, the facility was out of ratio on multiple days with one teacher and two or more unqualified aides which only permitted the supervision of up to 15 children. On some days, the Green and Blue classrooms had as many as 21 and 26 children, respectively, in those classrooms which poses an immediate health and safety risk to children in care.
