This poses a potential risk to the health and safety of children in care. 8 The facility will submit documentation showing that the required testing has been completed, including the laboratory results, to the Department by 10/8/26.

Based on interview and document reviewed, a 3-year old child was taken to emergency and received stitches. A fish tank that was placed on top of play refrigerator fell on top of child. It was noted that the refrigerator was not safely braced to the wall. LPAs also observed several shelves and aquariums that were not anchored to the wall.
